Contact drafts

The introduction and quote-request draft forms run in your browser. It prepares a message and does not send or store the form entries on a server. When you open a text-message draft, your device's messaging service handles the message. When you send it, Jacob receives the information you include. The personal-information review form sends your name, email, phone, ZIP code, contact permission, and quote selections to Resend, which processes the submission and emails it to Jacob at healthbyjacob0706@gmail.com. Do not include medical records or other sensitive documents. Resend’s own privacy terms apply to its processing. Quote selections and the estimate are carried in the review-page URL; avoid sharing that link if you prefer to keep those selections private.

Coverage calculator

The Your monthly estimate is calculated locally in your browser from a saved rate table. Your selections are not sent to the original quoting service. Choosing the text-message button prepares a draft in your messaging app; you decide whether to send it to Jacob.

Site operation

The site does not include advertising pixels or analytics scripts. The hosting provider may process technical information needed to serve and secure the site. Access to this private review site is managed by its hosting platform.

Sensitive information

Do not send medical records, Social Security numbers, or payment details through this form or an introductory text. Ask for an appropriate secure channel when needed.
